Report Your Injury to Your Employer
If you have been injured on the job or develop an occupational disease, it is important to notify your supervisor immediately. You must give notice to your employer if you have a workers’ compensation injury. This should be given immediately or as soon as reasonably possible following your injury.

Seek Appropriate Medical Treatment
It is important to obtain appropriate medical treatment when you have an injury. If your employer has a company doctor or company health care facility, it may be appropriate for you to go there. You may also need to receive treatment at an emergency room or other appropriate facility. Typically your employer will direct your medical care and tell you where to get treatment. If your employer will not do this, you can seek reasonable medical care yourself.

Notify Your Medical Provider That You Have a Work Injury
It is important that you communicate to your medical provider that you have a work injury so the medical provider can properly document this fact and so that the health care provider can properly bill workers’ compensation.

File Appropriate Claim Forms
An employee should file a claim form called a Form 18 with the Industrial Commission when the employee has a workers’ compensation claim. An attorney can assist you with this form or file the form for you if you desire.

Follow Your Physician’s Instructions for Treatment
Make sure that you follow and comply with all treatment recommendations made by your physician.
